The Byrd Clinic will be providing sports physicals on June 5th beginning at 7:30 AM.
The top 2/3 of the physical form page should be filled out and signed by the parent prior to the physical, and each athlete must bring $10 cash. These physicals are provided as a service for your convenience.
If you choose to use your own provider you must still have a physical completed and turned in before July 1st.
